Vital Pulp Therapy and Endo Essentials for GPs with Dr. Judy McIntyre

Speaker 0

You're into the pulp on a deep lesion.

Speaker 0

It's bleeding, and you have to decide whether that tooth needs a root canal or can stay vital.

Speaker 0

Dr.

Speaker 0

Judy McIntyre walks through exactly how she makes that call.

From why a diamond beats a carbide for a pulpotomy to the sodium hypochlorite pellet and the bioceramic sealer that follows.

Speaker 0

And then she gets in a cone beam.

Speaker 0

Why she now scans every case, the two canal canines that burned her, and why a whole head scan isn't an endo resolution scan.

It means a lot to both me and my family.

Speaker 0

With the recent shift toward vital pulp therapy and the explosion of bioceramic materials, how should general dentists realistically decide between performing a pulpotomy versus direct pulp capping versus committing to a full root canal on a mature tooth with deep caries?

Speaker 2

So a great question.

Speaker 2

For everything in endodontics and for everything really in dentistry, you know, the important part is knowing what your, like, your true abilities are and being confident in those abilities.

Speaker 2

So, obviously, for vital pulp therapy, there are a list of contraindications and then a a list of short, like, these are very optimal.

Speaker 2

So just addressing contraindications.

Speaker 2

So, obviously, one big contraindication for vital pulp therapy is the absence of preoperative symptoms, which frankly, like, having deep caries is kind...

Speaker 2

Like, goes hand in hand with preoperative symptoms.

Speaker 2

However, it is...

Speaker 2

It can exist, and it often does, especially in pediatric cases or, you know, a lot older teeth, right, that are heavily restored where it's still vital but they don't have pre op symptoms.

Speaker 2

So those are the, like, kind of the two big main categories that Vital Pulp Therapy is, like, working in under.

Speaker 2

A lot of clinicians do do vital pulp therapy, even endodontists, with, like, few or scant, like, meaning, like, not impressive pre op symptoms.

Speaker 2

So, like, well, it's a little cold sensitive.

Speaker 2

Let's try it.

Speaker 2

However, and this is just my personal opinion, once you're committing to a vital pulp therapy, right, you're losing the the ability to perform a future cold test.

Speaker 2

So if the tooth is going to fail or when it fails, right, really, you don't have really any way to test the health of this tooth until there's a periapical lucency.

Speaker 2

And, also, getting to that, right, through MTA or whatever you've capped it with is really, really difficult for an endodontist, let alone a general practitioner.

Speaker 2

It's very, very difficult to predictably find your way back and not create an iatrogenic defect.

Speaker 2

So to be very honest, vital pulp therapy, I do it a lot.

Speaker 2

But what I think happens a lot in dentistry, and we're all guilty of it, but it's the rushing.

Speaker 2

So when we're not explaining to patients, like, you've got to come back in six months.

Speaker 2

You've got to come back in one year, or, you know, you could end up with a jaw swelling or something worse and may even lose this tooth.

Speaker 2

Because we're talking about deep caries most often.

Speaker 2

So these these aren't really dentally compliant patients to begin with.

Speaker 2

So all of that is really, really tricky.

Speaker 2

So having these really heart to heart conversations without the pressure of rushing and time is super important for the successful viral pulp therapy cases.

Speaker 0

You're breaking up a lot of really important considerations with Vital Pulp Therapy.

Speaker 0

Things that sometimes we don't even always think about because I would consider this relatively new, or at least there's a lot more awareness around it.

Speaker 0

So let's actually backtrack for a second.

Speaker 0

Do you wanna explain what Vital Pulp Therapy is and perhaps what it's not?

Speaker 2

So it...

Speaker 2

It's actually a lot of things.

Speaker 2

Originally, Vital Pulp Therapy, you know, was designed for, I I believe, like, a child with deep caries and an open apex.

Speaker 2

You know?

Speaker 2

So it's like, okay.

Speaker 2

Like, you know, old school endodontics thirty years ago, they would have done, you know, apexification.

Speaker 2

But we know from the literature and from clinical cases that those are so high in failure.

Speaker 2

Like, managing a huge open apex, especially historically in two d, has been really, really hard.

Speaker 2

So the great thing about open apices and kids is their ability to heal.

Speaker 2

And, obviously, with open apices, you have all those great things apically like stem cells that are still in development.

Speaker 2

So the goal of vital pulp therapy, right, and let's just say, like, the the the lower premolar with an open apex is to get the caries out and then ottomy.

Speaker 2

So pulpotomy the pulp until with a usually highly recommended a diamond burn, not a carbide because the carbide will pull the entire pulp out.

Speaker 2

It'll engage it.

Speaker 2

So with a carbide, you're literally almost amputating slices and slices.

Speaker 2

Think of the the butcher at the meat...

Speaker 2

At the deli market.

Speaker 2

So you're amputating slices of the pulp until you get a stable, pulpal environment.

Speaker 2

Now what does that mean?

Speaker 2

What you're looking for is the pulp should not be gushing blood.

Speaker 2

It should not be a volcanic hemorrhagic site that's just a mess to maintain.

Speaker 2

And, of course, dammit, it's easy.

Speaker 2

All under rubber dam isolation.

Speaker 2

So you're doing this with the diamond bur until, like, you know, like a wound, like, when you were eight years old.

Speaker 2

You know?

Speaker 2

Like, wow.

Speaker 2

你……

You're...

Speaker 2

A chunk of your knee is missing, and you can see the blood, but it's not oozing.

Speaker 2

Like, it's just a lot of, like, fresh, raw skin.

Speaker 2

That's what it should look like.

Speaker 2

And from there, you place cotton pellets with sodium hypochlorite to disinfect, and you put it on from anywhere to two to five minutes, you know, with gentle firm pressure.

Speaker 2

And then you take it off and, you know, a a quick, you know, sterile saline rinse.

Speaker 2

And then you place something bioceramic, a putty, or a a white if it's, you know, premolar or anterior MTA or MTA on...

Speaker 2

If it's a molar, but we're talking about pre...

Speaker 2

This theoretical open apex premolar.

Speaker 2

I would probably do white on the child, and then you would either restore it or temper...

Speaker 2

Temporarily restore it with something firmer.

Speaker 2

I personally like Duo Temp by Coltine.

Speaker 2

It's kind of like an IRM, but dual cure resin reinforced.

Speaker 2

So it's it's just a really...

Speaker 2

It's nice to cut into.

Speaker 2

It doesn't get all goopy when you go get back into it like have it.

Speaker 2

So that is the true, like, historical vital pulp therapy.

Speaker 2

Now modern day, we...

Speaker 2

Modern day, like, let's fast forward.

Speaker 2

And, you know, kids often present with closed apex teeth.

Speaker 2

Or, you know, there's a heavily restored molar that on an older patient...

Speaker 2

I'm just gonna give two examples where, you know, it's still vital, but there's an absence of symptoms.

Speaker 2

So to avoid root canal therapy, you're going to, again, cold anymore, and then you can restore it, you know, however many months down the line that you feel necessary.

Speaker 2

So that is Vital Pulp Therapy in essence, which is super, super, super...

Speaker 2

Sounds like almost too good to be true and very easy, but it's a great, great alternative historically for these open apex teeth, whereas the epaxification was really not an easy procedure, not predictable, and not that successful long term.

Speaker 2

So that is Vital Pulp Therapy.

Speaker 0

Now what about in adult dentition with a closed apex?

Speaker 2

So for the, you know, the 12 year old kid on a six year molar with closed apices or the older patient on...

Speaker 2

That has a heavily restored whatever premolar molar.

Speaker 2

Again, in the absence of symptoms, you would do the exact same thing and just, you know, again hope for the best.

Speaker 2

And, you know, again, have that very clear and frank discussion that that this should be followed up routinely.

Speaker 0

So you bring up the most interesting consideration there, though, that we don't oftentimes think about is...

Speaker 0

So, yeah, now because the chamber's been filled with a bioceramic, we can no longer test

Speaker 2

它。

it.

Speaker 0

And so we have to wait really until we see an apical lesion.

Speaker 2

I mean, hopefully, right, that doesn't happen.

Speaker 2

但是……

But...

Speaker 2

So everybody should be clear that that may, number one.

Speaker 2

So proper expectations is always the key to success.

Speaker 2

But, also, here's where, like, it gets in the private practice setting a little bit more highly executional in terms of your systems.

Speaker 2

Because I'm always thinking about systems.

Speaker 2

So you're empowering your hygienist.

Speaker 2

Hey.

Speaker 2

You know, Susan's gonna be in in three months.

Speaker 2

Let's take a PA.

Speaker 2

You can go ahead.

Speaker 2

She'll be in to see you for her cleaning.

Speaker 2

Please make sure that you take a PA.

Speaker 2

And, you know, she can, the hygienist can percuss, then you can percuss also.

Speaker 2

So it should be percussion sensitive.

Speaker 2

It should be palpation sensitive.

Speaker 2

It obviously will be cold negative.

Speaker 2

And then, you know, that X-ray that that your hygienist, your team takes should show a healthy peri...

Speaker 2

牙周

Peri

Speaker 0

Along those lines as well, are you...

Speaker 0

Do you recommend that teeth that have been endo treated every year get a new PA?

Speaker 0

When I was in the air force, that was part of our clinic practice guidelines.

Speaker 0

It was something I did.

Speaker 0

I appreciate doing it.

Speaker 2

That's that's pure endo love, and I love, like, some of those military systems.

Speaker 2

Like, that's right up my alley because, right...

Speaker 2

I mean, I always think of myself as the Christmas Eve patient.

Speaker 2

So when I coach patients, that's what I say.

Speaker 2

Like, this is my life.

Speaker 2

I have three kids.

Speaker 2

Murphy's Law just backfires on me all the time.

Speaker 2

So I don't like to live a a life of gambling and risk.

Speaker 2

Like, you know, I don't wanna come into the Endodontist on Christmas Eve or Christmas Eve or when they're on vacation.

Speaker 2

So I am more proactive.

Speaker 2

So I'm all about systems to prevent.

Speaker 2

And, again, remember, endodontics, we prevent or treat apical periodontitis.

Speaker 2

So we don't want it to end up in a lesion.

Speaker 2

We wanna prevent that by doing it right the first time.

Speaker 2

So, absolutely, I I do love that system.

Speaker 2

And more importantly, conversely, I highly recommend PAs and testing before crowns are placed so that we...

Speaker 2

Know, like, we've talked about this on ELEVATED a ton of times.

Speaker 2

But people forget and, you know, insurance doesn't cover it because, right, like, FMX is every five years, which is so ridiculous because if you're placing a crown on an...

Speaker 2

用……

With...

Speaker 2

From an FMX that's four years old, like, there might be a perioglucency already on it.

Speaker 2

You know?

Speaker 0

Phenomenal point as well.

Speaker 0

Let's pivot a little bit here and talk about CBCTs.

Speaker 0

You gave a phenomenal lecture to Elevatedgp about, like, three d imaging, CBCTs.

Speaker 0

I wanna talk about that a little bit.

Speaker 0

So CBCTs are becoming much more common in general practice.

Speaker 0

Really, the adoption of it has dramatically increased.

Speaker 0

So for a GP doing their own Endo, what are the specific red flags or clinical scenarios where you'd, you'd say taking a per...

Speaker 0

Sorry.

Speaker 0

Excuse me.

Speaker 0

Where you'd say taking a preoperative CBCT is mandatory?

Speaker 2

好的。

Okay.

Speaker 2

So thank you, by the way, for that compliment.

Speaker 2

That was very kind.

Speaker 2

I do love CBCTs.

Speaker 2

And I think the reason during my journey to loving CBCTs is because I've eaten a ton of that humble pie from not taking CBCTs, and I don't like that taste.

Speaker 2

So, you know, on my journey and to answer your question, I've really come to the conclusion that sometimes, for me, I can't predict.

Speaker 2

I don't know when I'm going to need it because I've been burned so many times because, remember, I was practicing for a good ten plus years before I fully, fully, fully adopted and started using CBCT.

Speaker 2

So, you know, there's part of this, like, I've been doing this for many years.

Speaker 2

I've done tons of root canals without it.

Speaker 2

What do I need it for?

Speaker 2

But it's those hard gulps and surprises that humble you that make...

Speaker 2

Have made me move to a 100% CBCT.

Speaker 2

So if I take, right, like, my specialist practice, right, like, the the things that general dentists send me and then apply it to your question, I'll say.

Speaker 2

So anteriors, I would say, like, I'm hard pressed to say on incisors unless I see something very, very funny on two d, and I'm gonna come back to that.

Speaker 2

Canines, actually, I've been burned on canines, and I think I shared that on the presentation.

Speaker 2

Like, there are a lot of two rodent and two canal canines out there.

Speaker 2

And some of my orthodontist friends will, like, you know, because they scan or pano, and they'll just send me in the middle of the day.

Speaker 2

Like, there are a lot of two rooted two canal and canines out there.

Speaker 2

So, again, for the GP that thinks like, man, I'm gonna do canines and incisors, Just remember, they can have fast breaks, especially the lower anteriors.

Speaker 2

So so so when you...

Speaker 2

I wanted to come back to it.

Speaker 2

So when you're looking at two d, you really have to start with looking at two d very, very critically, which takes a lot of time, which general dentists complain they don't have a lot of time in their schedule.

Speaker 2

So it's really, really hard, again, because I don't practice as a general dentist, to imagine running like that during the day.

Speaker 2

So here are my recommendations.

Speaker 2

Number one, empower your team.

Speaker 2

Empower your hygienist and get better and find the time to really stare at those two d X rays for those fast breaks, for that little, little, little radiolucency that may be hiding a huge resorptive defect, all of those things, that takes time.

Speaker 2

And from the staring at the two d, only then can you get better at discerning what needs to be scanned with three d.

Speaker 2

Does that make sense?

Speaker 2

I would recommend all molars, obviously, all retreatants, all apicoes, and then the premolars with the fast breaks.

Speaker 2

For me, I love it and use it pretty sophisticated.

Speaker 2

So I plan my working length.

Speaker 2

I plan my access.

Speaker 2

And any kind of curve, I...

Speaker 2

On two d, you can appreciate.

Speaker 2

Is it curving this way and that way?

Speaker 2

You know what I'm saying?

Speaker 2

So I wanna know exactly what is expected of my file.

Speaker 2

Otherwise, if...

Speaker 2

Especially if there's a two degree curve, right, right, like this way and then, you know, pivots buckly or lingually, palatally, then that's gonna stress my file, and that's gonna risk file separation, which makes me really grumpy.

Speaker 2

So any kind of curves, any kind of fast breaks, which, again, are gonna...

Speaker 2

You're gonna be clued in by staring at your two d and being very, very suspicious always on two d, then you can start to develop your own systems with when to take the three d.

Speaker 2

And to that, one more caveat.

Speaker 2

Remember, a lot of general dentists have whole head cone beam in their offices and not EndoRes.

Speaker 2

So, you know, you're kind of comparing apples and oranges here.

Speaker 2

I wouldn't necessarily do...

Speaker 2

就是……

Be...

Speaker 2

I wouldn't be confident doing Endo based on a whole head CBCT.

Speaker 2

I need EndoRes CBCT, especially for resorption, especially for fast breaks, especially to visualize an m b two and where that's going.

Speaker 2

Like, is it a separate portal of exit?

Speaker 2

Does it join?

Speaker 2

All of those kind of details, you're not gonna get on full head cone beam.

Speaker 0

EndoRes。

Endo res.

Speaker 0

Tell me more about this.

Speaker 2

So, you know, usually, it's...

Speaker 2

So whole head, right, obviously takes the whole head.

Speaker 2

Endo res is usually just a higher resolution, you know, somewhere between 80 to 90 microns, I believe.

Speaker 2

I'm not an oral radiologist.

Speaker 2

I didn't study up on this.

Speaker 2

But, you know, obviously, you know, just a a little bit of a higher dose, higher resolution, and then, you know, allows you just to see everything.

Speaker 2

It's just so magical.

Speaker 0

So it's actually...

Speaker 0

This is a question that comes up in our group chat for a bit.

Speaker 0

But what CBCT?

Speaker 0

What do you like?

Speaker 0

What are you using?

Speaker 0

Again, this is not a sponsored episode at all, but people like to get practical.

Speaker 0

I mean, I like to be very practical.

Speaker 2

Great question.

Speaker 2

So in my office, I had Dexus.

Speaker 2

Used to be Gendex CBCT, which, you know, I loved.

Speaker 2

I always got a lot of compliments from other endodontists that were using Merida or Carestream about the clarity.

Speaker 2

So when I would present my cone beam presentation, a lot of endodontists in, right, were a picky bunch, would really complement the clarity, if they had a lot of views and familiarity with their machines.

Speaker 2

They're like, why is it so much clearer?

Speaker 2

So I do believe that, you know, DEXIS makes a great CBCT machine.

Speaker 2

Also, most sensors on the market are DEXIS sensors.

Speaker 2

So if you're familiar with DEXIS sensors, you're gonna get that same predictable image on the CBCT.

Speaker 2

That being said, I've also recently become more familiar with the Carestream product.

Speaker 2

我……

I'm...

Speaker 2

You know, Endodontists are not they...

Speaker 2

We are creatures of habit.

Speaker 2

So it's been difficult, but I am learning the Carestream.

Speaker 2

I don't have any familiarity with Merida.

Speaker 2

I've seen Merida images.

Speaker 2

I've seen Vault Tec images.

Speaker 2

And while I don't wanna bad mouth any product, what I will say is I personally do not like filters.

Speaker 2

So, like, a a lot...

Speaker 2

You know, like, you know, filters like Instagram.

Speaker 2

所以……

So...

Speaker 2

And, of course, we value filters.

Speaker 2

However, I don't value them on my images.

Speaker 2

Like, I wanna...

Speaker 2

Like, I am looking at the data and the images so rawly that I wanna zoom in and I wanna see, and I don't want it to be altered in any way.

Speaker 2

And a lot of...

Speaker 2

Some of the cone beam manufacturers will, in their algorithms, include the filters.

Speaker 2

So sometimes I'm just, like, almost, like, squeak squeak, you know, or or remember Clueless from the nineties?

Speaker 2

Like, oh, that's so Monet.

Speaker 2

You know?

Speaker 2

I I want the Monet off.

Speaker 2

Like, I wanna just see the raw images, but that's me just being a total nerd.

Speaker 0

Is there a is there a reason for that?

Speaker 0

I mean, are you...

Speaker 0

Is it distorting the image quality when you put a filter on it?

Speaker 2

You know, again, I'm not an oral radiologist.

Speaker 2

I just I just don't like it sometimes when...

Speaker 2

If I'm, like, splitting hairs over a portal of exit, it's almost like I wanna take the...

Speaker 2

That filter off.

Speaker 2

So I just wanna...

Speaker 2

I I don't wanna question what I'm seeing, and I feel like some of the filters do.

Speaker 2

However, to your first point, I think that it's supposed to be helpful and designed to be helpful for the average new practitioner learning CBCT.

Speaker 2

But, again, I don't know.

Speaker 2

I've never asked that great question, Melissa.

Speaker 2

But maybe one of the oral radiologists on ELEVATED can can expound on this before Mhmm.

Speaker 2

Later.

Speaker 0

Talk to us about minimally invasive endo, these minimally invasive tooth accesses.

Speaker 0

You know, is that a value add?

Speaker 0

Because I think a lot of GPs are hearing about this, and they say, you know, I don't know how to do this.

Speaker 0

So should I just never offer endo again?

Speaker 2

So I think, like, very similar to your first question, it's about knowing your ability.

Speaker 2

Now, again, I'm pretty conservative, but there are a lot of our colleagues that just kind of like to try a lot of new things.

Speaker 2

And, you know, kind of flex and push their limits.

Speaker 2

And that's cool too.

Speaker 2

We're we're we're all a bunch of people that have to, right, coexist, and we all have different personalities.

Speaker 2

I don't do ninja.

Speaker 2

Like, so...

Speaker 2

Like, I won't do the ninja accesses.

Speaker 2

And I'm so jealous of my colleagues who do.

Speaker 2

Like, serious.

Speaker 2

Like, FOMO.

Speaker 2

Like, I wish I could, but that's just me knowing my limit.

Speaker 2

Now will I do minimally invasive?

Speaker 2

There's been several, like, you know, accesses that I flaunted on Instagram because they're so small and tiny.

Speaker 2

But the moment that I feel that my my file insertion, my, you know, system, right, like, my tactile feedback.

Speaker 2

When I feel it torquing a little too much, that's when I know I need to modify my access, and I'm reaching my own limitation.

Speaker 2

It's not...

Speaker 2

So I need to enlarge that access for me, for my comfort level.

Speaker 2

Again, everybody is different.

Speaker 2

So do I love the movement?

Speaker 2

I love the theoretical movement of saving saving more dentin.

Speaker 2

You know, it...

Speaker 2

It's always been about saving teeth and saving more tooth structure for sure.

Speaker 2

However, again, if you're breaking every file trying to ninja or MIE, then something has to be compromised.

Speaker 0

I think that's really good and really constructive to point out, though.

Speaker 0

You know, this is something I talked a little bit about on the podcast.

Speaker 0

Sometimes I talk about when I lecture.

Speaker 0

Just being realistic with minimally invasive dentistry.

Speaker 0

If it's really not a service to the patient, has there been any value?

Speaker 0

So, you know, there are times where I'll get asked by dentists, hey.

Speaker 0

Should we still be doing crowns after Endo?

Speaker 0

Because after all, we wanna save tooth structure.

Speaker 0

And that's absolutely the right spirit.

Speaker 0

However, the odds ratio that the patient's gonna end up losing the tooth after Endo if we don't offer cuspal coverage is eleven times.

Speaker 0

So we have to be, I think, sober minded about being minimally invasive.

Speaker 2

I mean, this is just such a great exchange because I feel like, especially, like, on ELEVATED, like, the thing that I've learned participating the most in ELEVATED is I feel like sometimes we're just always, like, flowchart, flowchart, flowchart.

Speaker 2

Trying to get to the easiest answer, whereas we really have to think about it.

Speaker 2

And and in that thinking, in that step down to each next question on the flowchart, knowing our own limitations.

Speaker 2

Your and my flowchart may look very, very different.

Speaker 2

所以……

So...

Speaker 2

And being honest with yourself too.

Speaker 2

So, yeah, it's multifaceted for sure.

Speaker 2

I did also wanna circle back really quickly on the cone beam.

Speaker 2

So the great thing that I love about the DEXIS machine is that it'll allow you to take two quadrants on endo resolution, which is really helpful, and it also shows you the image that you're gonna capture before it actually does the scan so you can reposition.

Speaker 2

So a lot of times, you're not taking just a scan and then have to reposition and then taking another scan.

Speaker 2

The other thing is I always intentionally capture the opposing arch because, like, why would I just want one arch when I can have two?

Speaker 2

And there's oftentimes incidental findings, and I also talked about this on the lecture.

Speaker 2

There's a ton of incidental findings that you can find when you intentionally capture the opposing arch just for the the benefit of the patient and the referring dentist.

Speaker 2

Tons of findings.

Speaker 0

Let's talk about something a little controversial or a little difficult to talk about.

Speaker 0

And this is something that you and I have talked about just privately one on one where, you know, we'll...

Speaker 0

And again, I'm I'm a GP that does end up with the more I practice, the more I really am becoming more accepting and sober of my limitations and realizing that it is not a...

Speaker 0

It's not admirable to try to be a cowboy and do everything.

Speaker 0

You might think as a practitioner that you're really trying to push and expand your skill set, but the reality is, if that were your mom, is that what you would want done?

Speaker 0

So can we actually talk about this a little bit?

Speaker 0

Do you mind mean, you're you're this.

Speaker 0

能量。

Can't energy.

Speaker 0

Like, you're an educator, and you wanna help people to spread their wings.

Speaker 0

But at the same time, we've also just talked a little bit offline about how it's not always a service to patients when someone's being a cowboy.

Speaker 2

I mean, the past five to ten years, we've seen this super amazing movement to master dentistry where you have to become a master in, like, a master ceramatist and master color technician and a master aesthetic dentist.

Speaker 2

And, you know, if you do endo and implants in there too, then you're even more amazing.

Speaker 2

But, you know, it's really good to be...

Speaker 2

It's really hard to be good at all of those things.

Speaker 2

You know?

Speaker 2

So, you you know, it's hard ethically, like, for me to just say, like, if you wouldn't do this for your mom, then maybe you shouldn't be doing this.

Speaker 2

Because, right, like, otherwise, like, each person might answer differently.

Speaker 2

It depends on the relationship that you have with each of your specialists.

Speaker 2

The specialist should be encouraging you to do what you're comfortable doing as long as you're comfortable doing that.

Speaker 2

So, again, not being a cowboy and then having that relationship where you can trust them to take over and be that cowboy for you when you've reached your limitation.

Speaker 2

But at the same time, I also think I'm gonna just speak for Endo.

Speaker 2

I don't know how peri...

Speaker 2

Some periodontists feel, but, yeah, some endodontists might feel a little abused sometimes when, you know, Dr.

Speaker 2

Seibert only sends us, like, the retreatments and and the, you know, extraction cases and the difficult patients and the ladies who can't lean back and the pay...

Speaker 2

这个……

The...

Speaker 2

And the older gentlemen who can't open and all of those things.

Speaker 2

So, yeah, just remember, and you you know, I've I've done this hashtag for a very long time, love your endodontists too.

Speaker 2

So we're here to serve you.

Speaker 2

We're here to serve the patient and and not make you feel like a cowboy, but also support you as well.

Speaker 0

So what I am hearing in so many words is, hey.

Speaker 0

If you wanna have a good relationship with the endodontist, and you don't just wanna send them cases where you need to bail out or it's just a rough situation, If you wanna have a good reciprocal type of relationship, send them send them some nice, pretty straightforward cases or pretty fun cases.

Speaker 2

对。

Yes.

Speaker 2

当然。

Absolutely.

Speaker 2

You know, just remember that they exist, and they're not to be just abused.

Speaker 2

You know, a lot of my colleagues and I will just talk about a lot of the abuse that we feel sometimes, and and it it might not be intentional from the general dentist perspective.

Speaker 2

But, right, like, again, just remember we exist and we're there to have a relationship and not just be, you know, sent to all of the hard stuff.

Speaker 0

Very fair.

Speaker 0

Let's go back a little bit, though.

Speaker 0

We talked about avoiding being a cowboy or cowgirl.

Speaker 0

What are the elements of a well done root canal?

Speaker 0

Like, what are the things that if a GP's doing and doing their office that they should be looking for to say yes?

Speaker 0

Like, this was well done.

Speaker 2

So, you know, super great question.

Speaker 2

Back to the average general dentist stay.

Speaker 2

If they're rushing one...

Speaker 2

A rushing general dentist should not be doing Endo.

Speaker 0

I think they're rushing anybody.

Speaker 2

那……

That...

Speaker 2

Fair.

Speaker 2

好的。

Okay.

Speaker 2

Perfectly fair.

Speaker 2

是的。

Yes.

Speaker 2

Because if you're rushing...

Speaker 2

You can't think about doing each step properly, and I've given a lecture on how to set up for success.

Speaker 2

Setting up for Endo is no joke.

Speaker 2

Like, there is so much to set up for.

Speaker 2

And if you're rushing or if you're doing fly by the pants Endo, like, oh, I was doing an MOD, and now we're doing an Endo.

Speaker 2

And then your assistant is up half the time setting up while you're trying to start this Endo, that's really not setting up for success.

Speaker 2

And not only is the patient gonna feel it, but that endo's gonna be compromised.

Speaker 2

So setting up for success.

Speaker 2

Number one, rubber dam isolation.

Speaker 2

Number two, actually, in out of order.

Speaker 2

But number one, patient comfort and management.

Speaker 2

Like, is the patient comfortable in your chair?

Speaker 2

Because a lot of the...

Speaker 2

So patient comfort.

Speaker 2

So anesthesia.

Speaker 2

Because a lot of times, the patient doesn't wanna go see us.

Speaker 2

We're a stranger.

Speaker 2

We're scary.

Speaker 2

There's so much stigma around us.

Speaker 2

But I hear, like, probably 10% of the time, like, I wasn't numb.

Speaker 2

Or a general dentist will call me and be like, oh my gosh.

Speaker 2

I gave three carpioles.

Speaker 2

And I'm like, give more.

Speaker 2

You know?

Speaker 2

Like, there's so much fear, I think, especially in younger general dentist, younger practitioners about anesthesia.

Speaker 2

And it's like, it's Endo.

Speaker 2

你……

You...

Speaker 2

You've gotta give more.

Speaker 2

So number one, patient management and patient comfort.

Speaker 2

If you can't have the patient be comfortable and you aren't confident with anesthesia, that patient should be referred, number one.

Speaker 2

Because they're gonna be a lot more comfortable with a stranger and a scary, quote, endodontist than with inadequate anesthesia.

Speaker 2

Then I would say, obviously, confident rubber dam isolation.

Speaker 2

And then I would say, obviously, having the entire armamentarium, which means irrigation, file systems, and knowing how to fully obturate.

Speaker 2

So I have a lot of general dentists who have come to me for like, hey.

Speaker 2

This wasn't working when I was doing this Endo, or how do I irrigate, or how much should I have of sodium hypochlorite, and then what should I do?

Speaker 2

And if you're asking these questions and you've been doing Endo, you'd need a little bit more education before you do more Endo.

Speaker 2

A lot of times, what I'm finding is some offices will have only 30 cones or only 35 cones or whatever.

Speaker 2

But my point is if they only have, right, icing, like, one type of icing to decorate a cake, then you're not gonna be able to open a bakery.

Speaker 2

Similarly, if you only have one type of gutta percha and you don't know how to pivot with that one gutta percha, you shouldn't be doing Endo.

Speaker 2

That Endo.

Speaker 2

So oftentimes, they'll say, like, oh, I only have 25 cones or 30 cones.

Speaker 2

And I'm like, okay.

Speaker 2

So, let's...

Speaker 2

Let me show you how to use a gutta gauge.

Speaker 2

They're like, oh, I don't have that.

Speaker 2

And I'm like, well, then you're not using a full armamentarium to offer good treatment to your patient.

Speaker 2

You're you're not working with all of the flavors of icing that you should be.

Speaker 2

So all of that.

Speaker 2

And then, again, back to time and not rushing.

Speaker 2

So you need irrigation.

Speaker 2

You need lots of sodium hypochlorite.

Speaker 2

You need lots of, you know, sterile saline.

Speaker 2

I usually follow with sterile saline to rinse and then suction, paper points, all of that.

Speaker 2

Good sealer, confidence in your obturation.

Speaker 2

So if you have a cone that's doing that squiggly thing at the tip, which I hear often happens, or, you know, you're...

Speaker 2

You have, like, a...

Speaker 2

It looks like kind of like a a thin gutta percha, then, like, it's all fully obturated on top.

Speaker 2

It means you probably need to learn lateral or a hybrid to squish and fully condensed apical.

Speaker 2

You clearly have a mastery mastery of the middle and the coronal section of the root canal, but not of the apical.

Speaker 2

So that's where knowing how to pivot and knowing how to work gutta percha really is super important to offer a good root canal.

Speaker 2

So to answer your question, so absence of symptoms within three to five days, right, they should not be percussion sensitive.

Speaker 2

They should not have any lingering kind of discomfort.

Speaker 2

And, obviously, a well sealed, well, intimately filled radiopaque canal system.

Speaker 2

And then, obviously, if the core wasn't done that day, a very well sealed temporary until the core slash crown is placed, Cuspal coverage.

Speaker 0

I'm sure your answer to this is gonna be something along the lines of it depends.

Speaker 0

But do you prefer the single cone technique versus, like, warm vertical technique?

Speaker 0

And can you comment a little bit?

Speaker 0

Single cone technique.

Speaker 0

I mean, how viable is?

Speaker 2

So to be very honest, in the cases that I get, like, so calcified, I'm I'm just happy if I can get a single cone in sometimes.

Speaker 2

I still will do...

Speaker 2

Will try a little bit of lateral.

Speaker 2

And obviously, in palatal and distal canals, it's a hybrid technique for me.

Speaker 2

But, like, in a lot of the calcified canals that I get, I'm doing most of the time a one cone technique, especially on the buckles or the mesials of of my molar cases that are calcified.

Speaker 2

So there's nothing wrong with it.

Speaker 2

Again, the principles of endodontics are are always the same.

Speaker 2

It's disinfection to prevent...

Speaker 2

Disinfection and obturation to prevent or treat apical periodontitis.

Speaker 2

That is what we do.

Speaker 2

That's the goal of every single step.

Speaker 2

So I know my previous, like, answer was lengthy, but I felt like it was necessary so that really the discerning GP that's listening to this podcast can really think, and my answers will trigger.

Speaker 2

好的。

Okay.

Speaker 2

Am I doing that?

Speaker 2

好的。

Okay.

Speaker 2

Do I know?

Speaker 2

Oh, I don't know how to master my apical cone.

Speaker 2

Oh, my my apical cone does squiggle sometimes.

Speaker 2

I need more training on how to prevent the squiggling master cone.

Speaker 2

So, no, I don't...

Speaker 2

I mean, again, do what works for you as long as the principles of endodontics are adhered.

Speaker 2

So oftentimes, you know, I think that's why in endo residencies, you know, oftentimes, we have to learn lateral again after pre doc clinic.

Speaker 2

Because you need to learn how to troubleshoot every canal system.

Speaker 2

It's not a one size fits all.

Speaker 2

It's not a one system fits all.

Speaker 2

It's not a one cone technique for all canals.

Speaker 2

Every canal is different.

Speaker 2

So there's a huge advanced sophisticated understanding on how to get there.

Speaker 2

I know that was

Speaker 0

Can a explain a little bit, though, what the single cone technique is?

Speaker 0

Because the way it's been presented to me is really where you are just using a single piece of gutta percha that effectively is placed in there with a good bioceramic sealer.

Speaker 0

Is that a fair definition?

Speaker 2

That is a fair definition.

Speaker 2

I I guess, like, how's this?

Speaker 2

I don't teach it, so I don't know exactly what you were taught.

Speaker 2

So I just do what works for me.

Speaker 2

So I'm sorry if that sounds like a little lacking.

Speaker 2

不过……

But...

Speaker 2

So what I will do in calcified canals, and I'm just thinking, let's just say of a lower molar.

Speaker 2

Let's let's say there's two, a mesiobuccal and a mesiolingual and a fat distal.

Speaker 2

Okay?

Speaker 2

So I'm going to go in, and I'm going to start with negotiating with usually the cases that I get again, calcified canals, six or eight c files.

Speaker 2

And I hope...

Speaker 2

I try to get that down as far as I can.

Speaker 2

I'm gonna go in with an orifice shaper, and usually, that's like an o six taper and try to just make my life a little easier by orifice shaping, right, on all of three...

Speaker 2

These three canals.

Speaker 2

Then I'm gonna try to get to working length with a 10.

Speaker 2

If I can't, I'm gonna go back to my six and eight c files until I get to working length.

Speaker 2

And maybe I'll work in a little bit of the middle canal work in there too.

Speaker 2

But my point is, ultimately, what a single cone technique is for me is getting to working length with for me.

Speaker 2

And, every endodontist is different.

Speaker 2

I usually fill on o fours.

Speaker 2

So, like, if you look at my Instagram, look at my cases, I don't have the flared look root canal system because I believe in keeping as much peri radicular dentin as much as possible.

Speaker 2

So all of my cases, I finished to o fours consistently.

Speaker 2

There's no variable taper in the file system.

Speaker 2

It's just a standard o four.

Speaker 2

So on calcified canals, my goal is generally to get to 20 or twenty five zero four to working length.

Speaker 2

So then once I get to that, I might still orifice shape a little bit more, but generally, the whole system is an o four.

Speaker 2

So the one cone that I choose, the 20 or 25, whatever I've I've worked to, should match.

Speaker 2

It's a match.

Speaker 2

So the gutta percha should match the canal system that I've prepped to that exact size.

Speaker 2

So you put sealer on it.

Speaker 2

You put the cone in.

Speaker 2

One and done.

Speaker 2

Single cone technique.

Speaker 2

In distals, right, distals are often ribbon shaped.

Speaker 2

所以……

So...

Speaker 2

And palatals are often ribbon, rounder or ribbon shaped, but not uniformly the size of most gutta percha.

Speaker 2

So, generally, I find that that's where you have to pivot and do more hybrid, and single cone is less forgiving for me.

Speaker 2

So unless it's a really consistently calcified canal and you can get, like, a a...

Speaker 2

Let's just say on a palatal, a 30 o six or thirty five zero six, then maybe it would entertain, like, a 35 or thirty o six single cone technique.

Speaker 2

But it really...

Speaker 2

I have to feel like it's intimately engaging that entire file system and doesn't have any point of a butt or a bump in the canal system so that there won't be a void of gutta percha.

Speaker 2

Because sealer sealer shouldn't be your filling.

Speaker 2

Sealer sealer is the...

Speaker 2

Sealer...

Speaker 2

A lot of dentists approach me with that question.

Speaker 2

Like, won't the sealer fill count?

Speaker 2

We've seen it on elevated.

Speaker 2

Can't I add more sealer to make it look really opaque?

Speaker 2

You can, but sealer generally over a long term will wash out.

Speaker 2

And it's only the vehicle by the conduit for the gutta percha to kind of be fused in the system.

Speaker 2

The gutta percha is what should be in there.

Speaker 2

Because, also, remember, if you have to go back, right, and and...

Speaker 2

Because, right, we endodontists are always thinking of a way out.

Speaker 2

We've discussed this about posts on on elevated.

Speaker 2

If you have to take it out and you're using a bioceramic sealer, the bioceramic sealers tend to harden.

Speaker 2

So if you're gonna put the whole thing of bioceramic sealer in the canal system, you're gonna be working and possibly possibly have a higher tendency to perf.

Speaker 2

Whereas you need the gutta percha to hold that space because it's more easy.

Speaker 2

So that's the endodontist's way out.

Speaker 0

Well, you're gonna have to forgive me because we are almost out of time.

Speaker 0

We only got through half our questions, so we're just gonna have have you back.

Speaker 0

What are your closing remarks?

Speaker 2

I think the biggest thing would be, you know, recognizing one's own limitations, not rushing an endo, setting proper patient expectations, and having a really good relationship with your endodontist.
