播客 1009：预防复发性尿路感染

Good morning, everybody.

这可能不是世界上最吸引人的话题，但我们确实经常遇到。

Might not be the most exciting topic in the world, but something we see a fair amount of.

我们经常看到老年患者有尿路感染，尤其是老年女性。

So we see a lot of older people with UTIs, obviously, especially older females.

很多时候，我们就会想，好吧。

And, you know, a lot of times, you know, we think, okay.

觉得只需要再多开点抗生素就行。

You know, we just need more antibiotics.

有些人长期服用预防性抗生素，但最近出现了一些新的观点和指南，研究如何在急诊科就开始预防女性反复发作的尿路感染，其中一种方法是使用阴道雌激素。

Sometimes we see people who are on long term prophylactic antibiotics, but there's this I don't know if it's relatively recent idea, but there's some more guidelines and more studies looking at it of ways to prevent recurrent UTIs in female that we can start in the Emergency Department, and that's actually treatment with vaginal estrogen.

研究表明，这种方法能显著降低尿路感染复发的风险。

And it's been shown to pretty significantly reduce the risk of recurrent UTIs.

而且方法很简单。

And it's very simple.

只需要每周三次，每次一克就行。

It can be done just like one gram three times a week.

我们在这里就能开这个药。

Something we can prescribe here.

全身吸收的量极少。

It has very little systemic absorption.

通常非常安全，效果也很明显。

It's generally very safe, and it can be very effective.

像美国泌尿协会、老年病学会这些机构，都推荐用这个方法治疗反复发作的尿路感染。

So there's several societies like the American Urologic Association, Geriatric Associations that recommend this for recurrent UTIs.

我觉得急诊科的医生可能不太了解这个，但我们其实经常遇到这种情况。

I think something that maybe not a lot of people in the Emergency Department are aware of, but something we see a lot of.

如果患者和医生都没考虑到这一点，我们就应该提出来，通常也会建议使用。

So if it's something that we think is a the patient and the providers aren't thinking about, it's something we talk about and probably recommend.

所以我们能做的一件事，就是帮大家预防一些反复发作、总得回来就诊的问题。

So something we can do to actually maybe prevent some recurrent problems that people come back for.
