And speaking of, legends, we have a legendary conversation.
For those of you who are endodontists and some of you who are not, if you do not know the name Domenico Ricucci, you must have been just not paying attention to any literature or have had any academic experience whatsoever because when it comes to histopathology and even now, I would say, pulp therapy, biofilms, you know, just the whole etiology of root canals and failures, your name is all over, doctor Ricucci.
I just wanna welcome you and thank you so much for joining us today to just have some candid discussion about a few topics of patency.

Thank you.

It is my pleasure to be here with you and and the voices podcast.
Thank you for inviting me.
当然。
Of course.
对。
Yeah.
And and, you know, just to highlight a little bit about your background.
You know, you're you're an extensive researcher.
You've published upwards of over a 100 articles and publications and written chapters and books.
And so I'm always amazed at the passion that we have for our profession.
And so before we get into the discussion, maybe you can kind of just give us a little bit of history on your education and how you develop such a strong passion for what you do and how you do it at such a high level.
是的。
Yeah.
Basically, I'm I I need to tell you that I am an MD and graduated in general medicine because at my time dentistry was just a specialty of medicine.
So you had to be a general doctor six years before being a dentist another three years, which is too long probably.
Unfortunately, things have changing over the years.
没错。
Right.
And now we have also in Italy a dental faculty.
But it is good for the basis because you receive strong medical and biological basis and probably that was what prompted me to go deeper into the biology of the teeth and particularly about endodontics.
Then I met my mentor, Professor Langela.
I met him in 1987.
He was lecturing in Italy.
He was almost retiring.
Then we became friends.
I visited Connecticut many times, his laboratory and I could learn I could learn all the techniques to produce histologic sections.
Well, know the science of histology is I think an aspect, know, we kind of look at cases and we look at the three different tiers of gauging outcomes or success.
Course clinical symptoms, absence of pain, swelling, things of that nature.
Radigraphic, you know, we're really are we seeing any changes in the PDL, osseous transformation, resorption, things of that nature.
And then of course, histological.
And I think that is one of the reasons why we love your literature is because that's an element that oftentimes as a practicing clinician, we don't consider too much.
没错。
Yes.
And you're saying that a lot of it maybe is because your interest stems from your medical background and the understanding of that whole biological process.
没错。
Yes.
Although I have to admit that histology for dentist and general dentists particularly can be boring and can be difficult to follow.
But that is, let's say, my specialty.
I mean that histology, histologic sections of teeth or let me say, of endodontically treated teeth must be simple to read.
Everybody, every dentist should understand in order to be attractive.
But histology is nothing else than histo morphology, the morphology of the tissue in normality and in pathology.
So, after a minimum of training, every dentist should be able to follow the histologic slide because they show you just the morphology.
For example, talking about infection, histologic sections stained for bacteria can show you the morphology of the biofilm where the biofilm is located in the complexity of the root canal anatomy, the epicenter, the ramifications and the isthmus.
So it is easy to follow.
对的。
Yes.
没错。
Yeah.
With the staining, I can understand that it really kind of create makes it very clear.
You know, having this discussion is interesting, unfortunately, we can't really have the visual aids to kind of support that.
But we still want to just at least understand some of the concepts that have maybe have a controversial perspective, but we're here to learn and to look at ways that we can continue to save teeth through sound science and biology.
So that kind of brings me to I want to frame the discussion around the idea of patency.
对。
Yeah.
And maybe just kind of review the background.
What does that definition at least within root canal or within endodontics?
What does that really mean to you?
The idea of having a patent canal.
是的。
Yes.
Let me make a short introduction because the problem of apical patency is strictly correlated to the problem of the apical termination of endodontic instrumentation and of duration.
Let me say the endodontic work is split in two parts.
The let me call them biological a kind of biological school and, you know, the followers want to usually stay short of the radiographic apex.
Then there is another part and people want to aim at finishing all the procedures at the radiographic apex.
Was the first professor was Professor Schilder, Herbert Shilder
Although from
the followers of Shilder in time they have been modifying the concept of radiographic apex, they changed the terminology into the terminals of the canal, the apical terminals of the canal, meaning that the foramen can be anticipated, can be short of the radiographic apex and then they use this terminology.
So the problem of patents is strictly correlated because people who want to instrument the canal at the radiographic apex, they do not consider a crime invading with the tissue, with the instrument and with the obturation material the foramen, the foraminal part, which is already a part of the periodontal ligament.
So on the other part, the follower of the biological school want to terminate the procedure at the apical constriction, which is a point located always short of the radiographic apex.
So the story with patency originated with Professor Schilden in 1967 when his publication appeared about the three-dimensional feeling and then in 1974, there was the second publication about instrumentation.
So, Schilder started with a consideration that students or dentists with no experience during instrumentation, at that time there were no nickel titanium instrument, they were producing mud, dentin muds, PQ's, dentin chips and these were packed in the apical portion.
So they started with a certain working length and then after they realized that the working length was short because they couldn't reach the apical portion.
So in order to avoid this technical accident, technical problem, Schilder started to think that it would be good to recapitulate, to insist with a smaller instrument going up to the working length and even beyond that.
So, not consider a problem going with a small file, which was called patency file into the foramen and in order to prevent the package of debris.
That was the original idea.
But then successively, Buchanan has been defining precisely the concept of patency.
And and then again also the American Association of Endodontist recognized the terminology, the apical patency.
So apical patency is the use of a small hand file, could be a number eight, ten, 15, which is passively moved through the apical constriction without widening the constriction in order to prevent debris accumulation.
This is the definition.
So the patency file is used after each instrument during instrumentation.
Well I mean I love that history on the idea of the patency file and within you know our endodontic training we're often discussed the difference between the radiographic apex and the anatomic apex and I think you kind of touched on that and where that difference lies and we say usually based on the studies it lies usually 0.5 to maybe even one millimeter from the radiographic apex.
对。
Yep.
And so when we're talking about debris accumulation and maintaining that patency, is it still possible to employ that sort of technique and not disrupt or cause damage to the PDL or the apical tissues?
可以。
Yes.
It is a it is when you if you do apical patency, automatically the operator is invading the PDL.
Just automatically?
It's automatic because the you know, let me go back in terminology and you know that in some textbooks but still today the idle point were to terminate the endodontic procedure is considered the cementum dentin junction.
没错。
That's correct.
This is in text because it is believed that the transition from the pulp tissue to the periodontal ligament is located at the cementum dentin junction.
This is only partially true because the tissue in the apical pulp, the apical pulp is without odontoblast.
So if you look microscopically in the light microscope, the structure of the pulp in the apical portion and the periodontal ligament, they are the same.
It is a fibrous connective tissue.
So you will find fibroblast and collagen fibrous.
So there is not a clear difference between the pulp in the apical portion and the PDL.
But the most important point is that the cementum dentin junction is a histologic point.
So you can observe it only in the histologic section, so it is impossible to do that practically.
So I would invite all colleagues to abolish, to forget this terminology because it cannot be realized.
And then there is another important point.
If you cut a longitudinal section through the foramen, you will see that the cementum dentin junction is located on one side at a certain level.
On the opposite side, it is located at a different level, sometimes two, three millimeters.
So even though, let's imagine that we could identify clinically the junction, but then practically still it would remain impossible.
So please forget about that.
The only point that we can practically and realistically use as a termination are either the radiographic apex or the radiographic terminus of the canal or the apical constriction.
Apical constriction.
Which is some people would call it minor diameter,
which Of is course, at the end of the canal?
Yeah, but I question the terminology anatomical apex because there could be confusion.
Anatomical apex is not the constriction.
So, is important to know is that from the constriction in an apical direction, you have a space which is open externally.
It's like a funnel.
The foramen?
Yeah, the foramen.
It is different, which is a volume and not a point.
So, some clinicians claim that they want to terminate the procedure at the foramen, the foramen is a volume.
The foramen is the part which is between the major foramen, the apical external opening and the minor foramen.
So it is a volume.
And also then we come to the point, can our electronic machine today, apex locators, can, you know, identify exactly what is the frame and or the constriction?
Because we have very good, really reliable machine, but we have to learn how to use them because there is a confusion.
I you know, I'm glad we're having this discussion to clear up the confusion.
And we're gonna talk about all intents and purposes.
We'll think about the J.
Marita just for any discussion on and talk about those lines.
I wanted to get back to a point just so it's very clear for anyone listening.
Essentially, we fill to the radiographic apex within any tooth, it's considered long.
The filling would be considered long, correct?
If we fill to the radiographic apex.
没错。
Exactly.
You are always systematically beyond the limits of the canal but even Schilder in his publication admitted that.
Schilder said that by doing so the instruments are slightly beyond the limit of the root canal space.
But why should they insist that on the apical cross street, on the radiographic apex?
He was insisting because this is the point that you can always recognize in your radio Yes.
It's So all goes same back to imaging.
对。
Yeah.
Imaging for this reason.
So he admitted that the instruments and materials are beyond the limit, but he said that biologically this is not relevant.
This is what Schilder said at that time.
I I think even that that train of thought or that school of thought is still active today that maybe some would say we're destroying the apical anatomy but it's really not it's irrelevant.
Does does that make any sense?
Yeah, know, the problem is let me joke about that.
I say that if you decide to instrument and feel that the radiographic apex and so destroying the constriction, you are not killing your patient.
So the patient doesn't die because radiographically and clinically the cases can be absolutely successful in time, pay attention in the absence of infection.
没错。
Yes.
So this is the point.
So we have to think in terms of cumulative effect, what we are doing to the periodontal ligament to the cells.
So the mechanical insult, the mechanical damage together with the irrigation solutions and the material that we use for filling or for medication, we produce irritation of the tissue.
Irritation is an obvious consequence of every surgical cut.
So, we are cutting.
So, the problem is that if you stay at the apical constriction, you produce a severance of the tissue, you produce a wound which is minimal because of the constriction, the surface of this wound is minimal.
It is different if your instrument and feel at the radiographic apex.
So means beyond because you are producing more damage to the tissue.
Now, I do repeat, you are not killing the patient because in the absence of infection in time, irritation disappear, histologic inflammation will disappear.
And let's imagine you are using a zinc oxide eugenol cement sealer together with gutta percha and in time the irritant effect of the Eugenol because it is irritant will disappear and histologically what are we observing in time?
We are observing just some chronic inflammatory cells
surrounding the
granules of the particles of the cell remaining and or gut patches.
It is not progressively growing inflammation giving symptoms.
This is at a subclinical level.
So clinically, the tooth may be asymptomatic and radiographically you don't have any pediatric lesion.
So we consider these cases successful.
But you know, it's a basic principle of one ear.
Basic principle of surgery, keep the wound as small as possible.
对。
Yes.
Minimally engaging endodontics.
没错。
Exactly.
And I guess, you know, this is really fascinating discussion and we'll kind of touch on some more concepts in a bit.
But since we're talking about keeping the anatomy in its most natural form and keeping it as small as possible, What about the introduction of new advanced irrigation techniques?
GentleWave, lasers.
没错。
Yeah.
You know, and when we're employing this within that sort of with that sort of thinking that we want to keep the canal represented by its natural anatomy and keep that the constriction as small as possible.
What are your thoughts on some of these advanced irrigation technologies?
This is a very intriguing question, interesting.
New technologies based on laser are interesting and probably they will be the future.
The only problem is that there is no evidence concerning the possibility to completely debride the irregular spaces in the apical portion.
And when I say this, every endodontist understands that our problem is isthmus as in mesial root or mandibular molars or upper molars and so on in resorption areas, lateral canals and ramification.
So potentially, this machine and these technologies can help a lot in debriding these spaces from the tissue, particularly from necrotic and infected tissue.
This is our dream.
But unfortunately, at the moment, we have no evidence that this really happens, this really occurs.
So I always repeat that we would need histologic investigation of endodontically treated teeth and this is lacking today.
Well, I would say in The US and in Europe, this is difficult for ethical problems because treating teeth, doing this experiment in the mouth of patient is today impossible.
It is impossible also to perform these experiments in animals today.
对。
Yes.
But, you know, my recommendation is to use them, know, periodontally terminal teeth, for example, because in those cases you are not abusing patient, the tooth is lost.
The tooth test, let's imagine vertical and lateral mobility and is lost.
So I think that maybe this can be attempted to do this kind of experiments and to do the instrumentation, the procedure in vivo and then immediately instruct the tooth because that would be the final evidence whether this technology is removing all the tissue from the endodontic space or not.
This could be well, in the absence of in vivo studies could be done, these studies can be done also in extracted teeth, freshly extracted teeth, which is not the same, but still it is an approximation of what the machine really is doing.
But still, the final investigation is the histological investigation.
So serial sectioning of the root and the best sections are the cross section along the apical, middle and coronal turn.
对。
Yes.
对。
Yes.
And this is all very fascinating doctor Ricucci.
You know, I think one of the when we start to look at the studies that you've done with histology and if we're understanding the idea of patency and we're kind of getting rid of the idea of an anatomical apex and Yeah.
Kinda understanding where do we stop our apical preparation and how can we make that a consistent point with any within any case.
Because if we're saying that the the patency concept is really non existent or doesn't need to be applied to many cases, how exactly do we know where to stop our apical preparation and fill our cases?
The you know, all the the followers of the biological school, we aim at locating the apical constriction.
Once you located the apical constriction without going beyond
With the with the use of the apex locator
you're suggesting?
With use of the apex locator.
没错。
Yes.
Because the machine contrary to what is written in some brochures or some devices.
没错。
Yes.
The contemporary machines are very precise and they can tell you exactly the moment when the tip of an instrument is leaving the canal.
In other word, they can tell you with precision the point where the tip of an instrument is no longer surrounded by dentin.
Every electronic machine today can tell you this, although they are set in a different way.
So if you know how your machine, the machine that you have in your office works and if you know that the machine is telling that point, this is the constriction.
尽管
Even though the
The dual frequency, the difference between the PDL, the frequency and of course the
This is very precise because they tell you at this point you are independent of the ligaments.
You So know you stop there, you record precisely that point and you bring a series of instruments with increasing sizes at the same point.
So, is very important today because the apical preparation is better done if you use your hands instead of rotary instruments.
So, rotary instrumentation is good but every clinician knows how difficult is it to maintain the
same Very difficult,
是的。
yes.
Because sometimes The reference point is
Yeah, tend to screw inside and go beyond.
You bring a certain number of instruments of increasing sizes at the same point and you are carving an apical cola at that point.
This is very useful in the moment of filling because it helps you to prevent overfilling.
Even you were saying overfilling considering bio ceramic sealers as being traumatic to the
Yeah, at least you PEL
作为
as
can control your master point at that point we know then we need a fluid part of the filling which is a sealer, conventional sealer or the modern bio ceramic sealers.
And we know that a minimal amount of fluid material is always beyond the corner.
没错。
That's correct.
So we don't consider this a tragedy.
The problem is when you use, for example, warm good aperture or softened good for example, thermal field or similar systems and then you have extrusion not only with silhouette but also of gutta percha.
It's a problem of the amount because silhouette, conventional zinc oxide usually in silhouette are restored pretty easily and quicker compared to the resin based.
They remain in the periapical tissue for a long time.
And this could be a problem when they remain for a long time.
We know very little today about the bio ceramic, they are always sold less easily than in the past.
But still the problem remains.
So if you create a corner, an apical stone, this is a great clinical advantage.
That is a clinical advantage.
And when we talk about different sealers, you know, epoxy resin or any sort of the bio ceramic, we know that the bio ceramic, yes, is a little bit more biocompatible, you know, less shrinkage.
It has a lot of properties that we value.
Some would even say it has some osteogenic, I guess, sort of capabilities as well.
Is there a difference though when we talk about where to define our apical preparation in a vital case versus a necrotic?
So when does the diagnosis, so to speak, if we're looking at diagnosis as necrotic pulp, vitality based on how we conventionally assess for vitality with our pulp testing or EPT, do we find that there is any difference in where we should stop our apical preparation based on the diagnosis of the tooth?
Let's start with the vital cases that you mentioned.
When you have vital pulp from the orifice to go in an apical direction, you have no bacteria.
So this is a great advantage for the clinician because you are not facing infection.
So when you go to the apical to the apical termination, there is biological and not a single reason to destroy the tissue in the ramifications on the on or in a you know in the apical portion of the canal because the tissue is vital.
So producing
Can I stop you?
Just to clarify, so we would we want to keep the PDL intact to support the vital tissue in the apical third.
是的。
Yes.
Got it.
To keep vitality of the tissue in the apical foramen or foramina and in the ramification.
So in vital cases, patency is biologically against any let me rephrase, is against any biological evidence.
You don't need to destroy that tissue because you have to think that circulation for the pulp in the ramifications, in the irregularities, the circulation originates from the periodontal ligament.
So the vessels go from outside to inside.
没错。
Yes.
So the vitality maintained over the years in the absence of infection.
And this is very important because removing completely 100% of the pulp tissue in the endodontic space is impossible.
And we can say that loud and clear.
Say it one more time, doc.
It's really impossible.
In vital cases to make an instrumentation which is invading the prefrontal tissue is not biologically based.
Now, obviously you ask me how about necrotic cases in which bacteria and infection can reach the apical portion of the canal.
Let me tell you that for a long time the tissue in the apical portion, even when you have a large periapical lesion may be vital.
This is poorly understood in general and university do not teach that.
But the reason is very simple because when you have a severe infection in the pulp crown, in the coronal middle third, the toxins or bacteria and byproducts are transported, diffused through the connective tissue into the periapical space where an immune reaction is established.
So practically, this is a great advantage for the clinician.
We don't know that, but it is a great advantage because despite necrosis can be sometimes invading a lot of tissue, a lot of the pulp space, we succeed, we have a high success rate also in necrotic.
That's correct, yes.
And the reason is exactly this, not in all cases necrosis has been reaching completely the endodontic space including irregularities and ramification.
Okay?
So this is the reason why we're here.
But I do admit that in the final stages of the pulp degeneration process, the process can invade also apical ramifications.
And that then practically, let's imagine that you can reach with your patency file one of those ramifications and you can eventually also disrupt the biofilm present in that ramification.
But the question then comes in how about the other ramifications?
没错。
That's correct.
Yes, the apical anatomy, the complexity of the apical anatomy is well known.
对。
Yes.
So everybody knows that.
So it is meaningless that you disrupt the biofilm in one aromification and
not in
the other.
Yes, because clinically and radiographically even using CBCT, are not aware of the complexity of the anatomy in the epical curve.
So still it is not an advantage.
But let me tell you, there is another risk.
The risk is that by doing this, you penetrating through the constriction carries the risk of transportation of bacteria, pieces of bacteria by film from the canal into the perietic tissue.
And Can I stop you
right there?
That's a good concept.
The transportation of bacteria.
But just so I'm clear, in a lot of your histological studies and presentations we've seen that usually within I guess the coronal two thirds of the tooth, that's where most of the bacteria in necrotic tissue will be housed.
Correct?
是的。
Yes.
And you're saying that even in the presence of what we call a periapical lesion
是的。
Yes.
That occurs because of the transfusion of the byproducts of the bacteria, not necessarily that the actual periapical lesion is infected with bacteria.
No, yes.
I'm not talking about this extreme cases.
So if you
have if you have vital tissue there, when you reach the vital tissue usually the clinician is able to understand that because when you start to extract from the canal a tissue that is blood filled, continuous, you recognize the vital tissue.
And this is an important information for the clinician.
But there will be cases, cases with long standing necrosis where you go deeper, you reach the apical portion and you extract, if you observe on the tip of your instrument what you are extracting, you can see that the tissue is sometimes necrotic.
明白了
Got
it.
Meaning that necrosis has already reached reached the 8% portion of the gland.
Those are the cases in which the risk can be seen carrying bacteria beyond the limit of the gland and creating an extra radicular infection.
Now, if you push bacteria there, not always this is a clinical problem because you may have a flare up, you may have a postoperative flare up.
But sometimes this is managed by the immune system.
But don't forget that we have a particular species of bacteria, which is actinomyces bacteria, which are resistant when they live in periapical tissue.
So this is the characteristic of those bacteria.
They form colonies which can resist phagocytosis.
So when they are in the periapical tissue they can survive, multiply and this is one of the reason for failures.
We call that extra radicular infection.
So actinomyces is usually present in the canal.
And when you do your patents in necrotic cases with necrosis up to final portion of the canal, you have the risk to query that.
You know, I actually have had a case, just through the histological sample where we found out that it was actinomycosis and that's why it was so resistant.

And now we're back live on the floor at AAE twenty three in Chicago.
So I guess now since we're talking about those cases where we do not have success and we actually have failure, Now we're talking about another option of exploring surgery to actually address those apical ramifications where the biofilm exists.
对。
Yes.
And so what are your thoughts on you know surgery and apical surgery?
It's necessary and mandatory sometimes.
是的。
Yes.
Every endodontist should be able to do apical microsurgery today.
So we fail in a certain number of cases.
Success rate when you start with this is important to repeat, when we start with conditions of vital pulp, our success rate in the hand of a specialist is ninety five percent, ninety six percent, a very high success rate.
When you start with a periapical lesion and necrotic pulp, success rate decreases to eighty three percent, eighty four percent.
Almost ten percent, yes.
Difference is easily explainable in terms of location of those bacteria.
Those bacteria are located intradicularly, I mean, in the endodontic space in areas that are distant from the main canal and not reachable by arrogance
器械。
instruments.
所以
So
those cases fail.
And this is the main reason for failure.
So the number one reason is the intradicular persistence of bacteria.
对。
Yes.
But then don't forget we have also the extradicular infection and I mentioned before the actinomyces but there is another one which is the presence of a biofilm on the external surface of the root.
Now in my end in Sicera research about the infection, about the biofilms, we found that this is alarming that in cases with periapical radiolucencies, the presence of an extra radicular biofilm formed on the external apical surface occurs in six percent of the cases, which seems to be a small number, but it is not small.
So it means that practically when you do your treatment and you perform your instrumentation in the best possible way at the highest technical level, but you cannot eradicate the infection, the biofilm on the X-ray.
On the X-ray.
Ridiculous surface.
And this is, at this point, is easily understandable why we do the orthograde treatment, we fail, we do the surgery and the case succeeds because we are physically removing infections in those areas that we could not reach with the orthograde treatment.
And when we talk about those places that are inaccessible on the extradicular surface, I think we kinda even have to draw from some of the classic literature and I heard you mention the Saqqara, but also when we look at maybe Nare discussing a true cis versus a bassist.
And the idea that there's no communication with the extra radicular portion of the root and the canal.
So that's why so many of the cases may fail is because it's actually cystic in nature.
是的。
Yeah.
You are now opening another question.
I you about it.
I I am I'm known to be the men of the controversy.
对。
Yes.
See.
I love this conversation because Yeah.
You know, in residency, just looking at your histological sections and knowing the literature, it just it does make so much sense.
How can you still address the biofilm that's on the extra radicular surface of the tooth?
And so so patency so the concept of patency was almost kinda counterintuitive.
对。
Yeah.
And so I guess it's always just been an issue of when we talk about, oh, okay, the minor constriction and then the difference with the like you said, the volume of the the foramen.
Morita Root ZX II。
Just, I guess, let's have bring the conversation back to let's assume that we're using a J Merida Root ZX two.
Are we taking it to the blue line, the middle of the green, to the red?
How do we know based on this technology where we should stop our apical preparation?
Because there's, know, I think there's just so much variation in how do we know where we should stop.
We're Every manufacturer makes different claims.
对。
Yes.
Because, know, they want to please everybody.
All all the schools, so they cannot be so precise.
If you are talking about the Morita because it is the most popular machine in the world, they have I think around 70% of the world market.
是的。
Yes.
They recommend to use it.
Oh, no, they don't recommend.
They claim that the cost reduction is located at a zero reading point, which is in the middle of the green scale.
是的。
Yes.
So I since I know the machine very well, I can tell you that this is not correct because the cross section to me is located a little bit before when you are at the end of the blue scale.
The end of the But blue everybody can do that using an alginate model and extract the teeth.
You can see yourself.
You block the instrument and extract and observe under microscope to see where the tip of your instrument is located.
So I think that everybody should make a setting of his own machine because then you can understand.
So for me, the co trick is at the end of the blue scale.
If you go beyond, even at the middle of the green scale or worse at the pink scale, are deep into the periodontal ligament.
You are beyond the limit.
But let me go back to the question that you raised because it is just a few words to say about the story of the true cyst.
The true cyst is believed to be one reason of root canal treatment failure.
是的。
Yes.
So, we have to say something.
First of all, nobody can make this diagnosis based on the regulators or even CBCT because a cyst is a histologic diagnosis.
I'm so glad you clarified that.
We don't see it.
It's very important because the reasoning is usually the lesion particularly by the general dentist, there is a lesion.
The lesion is big.
The margins are sclerotic or corticated so it is assist.
If it is assist, it doesn't heal with, cannot heal with orthogrid treatment so I have to send the lesion to the surgeon and this is patently wrong and our patients are suffering from this.
So first of all, we cannot identify the lesion based on the endodontic.
So it is a cyst.
You, clinician, you can say it is of endodontic origin or not.
This is important.
If it is of endodontic origin, you, the endodontist are the best specialist to treat this pathology.
And you treat with the orthograde treatment, you control the infection and the lesion must heal.
Now, let me make a consideration which could be useful.
If it is true that the true lesion doesn't heal, automatically it is no longer an inflammatory disease.
It became a neoplastic disease which is not true.
So, in terms of general pathology, even the true cyst is not a neoplastic disease, it is an inflammatory disease.
And if the disease starts as an inflammatory disease, when you control the etiology of the lesion, every lesion should heal.
So in term of general pathology, it is impossible that a true cyst doesn't heal.
Then we have another evidence for that.
And the evidence originates from the dozens of biopsies that we took of big lesion, not healing.
So, I found personally many times that a lesion was a true cyst morphologically.
Okay, but at the same time I found that there was infection in the apical portion.
So you cannot blame the presence of a true cyst as being responsible for the persistence of the lesion, but it is the infection.
In other words, make it simple, I would be the first one to believe that the true cyst is a region of failure.
If you take the biopsy, you cut serial section, it is a true cyst and at the same time there is absence of infection in the epical third.
Then this would be the evidence that the lesion didn't heal because it was a true cyst.
But this I have never, never found until now.
Until now.
So the true cyst
是
is an
endodontic disease, it's induced by endodontic infection, You control the infection and the cyst will heal.
And the cyst will heal.
Wow.
You're getting pretty deep on this doctor Ricucci, but this is this is very good stuff.
So so I guess we just wanna now take some of that that theory and reduce it to the practicality because, you know, we've we've had some good conversation.
So now let's talk about the ability to obturate.
And we discussed, you know, is patency valid or is it not?
The apical ramifications and the positioning of the bacteria or necrotic tissue mainly within absent, I should say, from the apical third.
So now how are you actually sealing these cases and what is the best technique based on your studies and empirical data to fill these cases with the sealer based obturation?
If I understand well, you mean about these complex spaces that we cannot
没错。
That's correct.
I wouldn't recommend to sell them.
I know that this is against the mainstream.
This is it goes in another direction.
Yeah, you are opening another problem, the problem of lateral canals.
Again, it is considered today by a certain part of the endodontic profession that if you feel lateral canals, is excellent.
I love
seeing lateral canals.
Mean, it's it it an endodontist, feel good.
Now, the feeling
She agrees.
对。
Yeah.
The the feeling yeah.
Every every clinician, when see a lateral canal, he's injecting.
I use the word inject and not filled the Yes, exactly.
In the radiograph, would be happy or how good I am and you feel better.
Yes, but this is a psychological problem that we should solve sooner or later because unfortunately I have extensive studies on that Injected by or evident in the radiogram doesn't mean filled.
It doesn't mean filled.
It means that
Filled three dimensionally.
Three dimensionally.
Okay.
Not only, but it doesn't mean that the tissue which was there is being removed.
So if there is a vital tissue there, when you squeeze the material, when you do your filling, you apply some pressure and if the material is fluid enough, it will penetrate into this ramification giving you the radiographic impression that the space is sealed.
But if you cut sections, then we have a lot in patient, in human, where the material was apparent geographically, then you find that the material, the filling material is just intermixed with pulp remnants of pulp tissue, partially necrotic, partially vital when the case was vital.
So you are not filling the space and the tissue is partly necrotized.
When you have necrotic cases and necrotic cases are filled, if lateral canals are filled more easily, no?
Because there is no tissue.
No tissue.
The tissue is different, different consistency of the tissue.
Then the same story, you are squeezing material in a space that was not disinfected because disinfection is the problem.
I know and I do admit it.
In those cases, extreme cases where you have bacteria into ramification, it would be highly desirable to get rid of those bacteria to remove them.
当然。
Of course.
But we cannot.
That's an etiologic agent.
You complete your instrumentation and bugs are still there.
Then you fill and you squeeze material into ramification.
This material will not kill those bacteria.
So we have a lot of evidence, histologic evidence, which is published that we cannot solve the problem of infection in those spaces.
So I would, let's say, diminish the importance of filling bladder canal.
So I would say finally, well, it may happen because it is the fluid part, it is not a good thing and it is not a bad thing.
It happens.
It happens.
So, we accept that but I don't jump for the happiness when I see that one lateral canal is being injected in my opinion.
Because I know that the problem is not to fill, the problem is to clean those spaces.
So so then if if that's the thinking that maybe we're not filling or sealing those ramifications, Are we ever able to eradicate bacteria, biofilm from those apical ramifications in a consistent manner?
Not today.
Don't have the technology to do that today.
That's what we want to hope find
that this new technology will prove to be efficient in what they promised to do.
Because this would be a considerable change in our profession, but at the moment I can tell you that we cannot.
That we cannot.
And then we continue to have ten-fifteen percent of failures in necrotic cases which is we have to accept.
We're going to have to have some comparative studies with that.
没错。
Yeah.
To really see where the technology is taking us because this is a technological revolution as far as endodontics is concerned, whether we're talking about the ERG YAG or YSTG, you know, sonic irrigation.
But if we're not seeing a shift in the outcomes, an improvement in the outcomes, specifically necrotic cases, so what are we really doing?
So it's really, I guess, like you mentioned and you started the conversation, it's an understanding that you gained from being trained as a medical doctor of the biological processes of which we're dealing with in cases of irreversible pulpitis and necrosis where we're trying to eliminate or prevent apical periodontitis.
Exactly.
The problem is infection today and one practical advice for dentist, for endodontist is that you have to try to understand how long the pulp has been necrotic there.
Because sometimes you know the patient doesn't have pain so you say okay, I don't have pain, I don't treat.
No, we have to push the patient to treat the canal as soon as possible because if you wait, you wait one, two years, you are giving time to bacteria to colonize distant areas and you are worsening the prognosis.
This is a practical advice.
So as you said, endodontics is we want to prevent or treat apical periodontitis and it is better to prevent that.
对。
Yes.
So prevent that necrosis progresses deeply into the root canal space.
And doc, just while I have you here, can you just please make it very clear that a lesion on a radiograph does not indicate that we should be prescribing antibiotics in the absence of systemic involvement.
I get so many colleagues that send patients on antibiotics just because they're looking at what they see, some osseous resorption, and they say automatically let's go ahead and give them an antibiotic.
And it's just, you know, I just don't feel that that's validated in any of the literature, any of the research unless there's obviously some sort
没错。
of Yeah.
Systemic Because you give me the opportunity to stress that we are abusing antibiotics.
Thank
you.
So, and and this is a logical concept more often than not if you analyze microbiologically histobacteriological pre epical lesion ninety five percent of the cases this is free from bacteria.
对。
Yes.
And thanks God that the lesion exists there because in the absence of a pre apical lesion, everybody would die for septicemia or bacteroid.
So the the the goal, the biological goal of the lesion is to block the aggression of bacteria from the canal to reach the general circulation and distant vital organs.
So this equilibrium so this is something cast in the stone.
The re epic lesion is not in fact normal.
There will be conditions in which the equilibrium between the immune system and the bacteria is broken.
没错。
Right.
And the most common condition is the abscess, the clinical abscess.
When the patient is swelling, in that moment bacteria are in large number invading periapical tissue.
But this condition is transient and even though you give or not antibiotics to the patient, sooner or later immune cells will win the battle, okay, and retreat the bacteria into the canal.
So this is a transient situation.
Apart from this situation, which is very common, the clinical abscess, bacteria can be in periapical tissue only in under two circumstances.
One is the formation of a biofilm on the external apical surface.
The other one is the presence of actinomyces which can lead a free inside the inflammatory tissue.
These are the only two conditions.
But the idea of giving antibiotics to reach these bacteria is false because bacteria are not there with few exceptions.
With few exceptions.
Doctor, this has been just an amazing experience.
