Welcome to yourxraytech podcast.

Speaker 0

In this podcast, we talk about radiology.

Speaker 0

My name is Jason.

Speaker 0

I'm an x-ray technologist.

Speaker 0

And today I got a guest, Lazy Bones Radiology.

Speaker 0

He's going to talk about interventional radiology.

Speaker 0

He's going talk about x rays and a few other things.

Speaker 0

It's going to be a great interview, a great podcast.

Speaker 0

I'm looking forward to it.

Speaker 0

Let's get it started.

Speaker 1

My name is Carlos Buitrago-Pinzon.

Speaker 1

I'm the creator of Lazy Bones Radiology.

Speaker 1

I work in Interventional Radiology, for the past five years.

Speaker 1

I went straight from X-ray school into Interventional Radiology.

Speaker 1

I was very blessed because my hospital, they needed the, IR techs.

Speaker 1

So when I was a student, they hired me right on.

Speaker 1

I got on the job training and I was able to get my certification.

Speaker 1

After, once I passed my x-ray board, seven months later, I passed my IR boards.

Speaker 1

It was pretty fast paced, but I love it every step of the way.

Speaker 0

And I do want to talk about the path of going from x rays to interventional radiology in a few minutes, but let's bring it back a little bit.

Speaker 0

What is interventional radiology?

Speaker 0

Like how would you sum it up in your words?

Speaker 1

So interventional radiology is pretty much we work with the vascular system of the entire body except the heart.

Speaker 1

Usually that's for cath lab, from the brain all the way down to your toes, arteries, veins, organ.

Speaker 1

We're able to put filters, coils, anything that deals with the vascular system or the organs, we are able to, work on them.

Speaker 0

Wow.

Speaker 0

That's interesting.

Speaker 0

And there's so many options, right?

Speaker 0

And interventional radiology is one of those.

Speaker 0

Options.

Speaker 0

What is the most common procedure in interventional radiology?

Speaker 1

So it depends on what the type of hospital you're at.

Speaker 1

So there's some hospitals that are specialized.

Speaker 1

Like for example, some are neuro based, so they deal with more strokes or embolizations of like aneurysms inside the brain.

Speaker 1

Some hospitals are more like dialysis clinic where you have like fistulas, you have grafts, you have dialysis catheters, and then some other specialties, like for example, they just do open heart or thoracic surgery.

Speaker 1

Interventional can help as well.

Speaker 1

Some are just everything, which is my hospital.

Speaker 1

We do pretty much everything.

Speaker 0

On average, how long would you say an IR procedure take?

Speaker 1

Well, it depends on the procedure that you're doing.

Speaker 1

So for example, we have something called a TIPS procedure or transjugular intrahepatic portal systemic shunt, which is a very big word or very long process that we put a stent from the portal system into the inferior vena cava in order to bypass the blood flow.

Speaker 1

That could be a thirty minute or that could be a six hour case.

Speaker 1

It just depends on how lucky can you get.

Speaker 1

And sometimes the fistula is the same way, ten minutes or you're there for like four hours.

Speaker 1

Triple As or yeah, triple As, the longest I've ever done was like almost eight hours long.

Speaker 1

It was like a very complex anatomy.

Speaker 1

You got to do a bunch of different stents, coiling.

Speaker 1

You got to do a whole mapping of the entire aorta.

Speaker 1

That was like an eight hour case.

Speaker 1

That was pretty, pretty hardcore.

Speaker 1

But the thing about Interventional Radiology, we work as a team.

Speaker 1

So you have the radiologist right next to you.

Speaker 1

You're the tech.

Speaker 1

You have the scrub, the circulator, you have a whole team of nurses.

Speaker 1

It's an entire team and squad that deals with just one patient.

Speaker 1

And that's one thing I love about Interventional Radiology.

Speaker 0

With Interventional Radiology, you're part of a team and it's a very tight knit team, right?

Speaker 0

I mean, X-raytech, you're part of a team as well, but this one's like, you're working very close, you know, and pretty much everybody knows everybody.

Speaker 0

没错。

Absolutely.

Speaker 0

So can you kind of speak on that on the team, maybe your role and, you know, your experiences working in interventional radiology as a team, a member of the

Speaker 1

So when I first started as a student, I was just circulating.

Speaker 1

So you have to know what wires to get, the catheters, because every single you have different types of wires, different types of diameters.

Speaker 1

The catheters have different shapes on how we are able to access the vessels.

Speaker 1

So when you first start, you have to know the product.

Speaker 1

Why do we use it?

Speaker 1

How do we use it?

Speaker 1

And like the stuff, because you have to be as a tech, you have to know what step is next.

Speaker 1

That way you're ahead of the physician.

Speaker 1

And that way you're able to hand off what the physician needs.

Speaker 1

That's one thing that when you first start, you have to circulate.

Speaker 1

And then after you're comfortable, the next step is scrubbing in with the actual physician.

Speaker 1

You are literally the assistant of the physician in my hospital.

Speaker 1

And you have to know the steps, also the anatomy, because you're assisting the physician.

Speaker 1

So the physician, for example, if you're looking at a bleed, you have to know where you're at, what vessels, like, okay, this type of wire goes with this type of catheter.

Speaker 1

Because the physician goes, I need this.

Speaker 1

You have to know how to prep it, how to make sure everything's flowing correctly.

Speaker 1

That way he's able to do his job and I'm able to keep the whole flow going.

Speaker 0

So what is a day in the life as a technologist that works in interventional radiologists?

Speaker 0

What does that look like?

Speaker 1

So you wake up before the sun rises, you have to get it, get in there, turn on all the machines, the big C arm, all the suites, make sure everything's nice and stocked.

Speaker 1

Every day we have to do inventory because we use so much, inventory, coils, balloons, catheters.

Speaker 1

We have to make sure everything's up to par.

Speaker 1

That way when, if an emergency presents itself, we're able to treat it with no, hey, we're missing something.

Speaker 1

So that's one thing that we do every morning, actually breakfast before all the cases start piling up, pretty much get all the patients that we have to get.

Speaker 1

We check their labs, make sure everything's in proper parameters.

Speaker 1

If anything is out of the ordinary, we have to call our physician like, hey, what do you want to do with patient?

Speaker 1

Like pretty much we do all the screening before the patient even gets there.

Speaker 1

Also the nurses come.

Speaker 1

They also make sure all the medications, everything everything's prepped before the patient even gets into the actual suite.

Speaker 1

Because once the patient gets there, it's showtime.

Speaker 1

There's no, oh, I need this.

Speaker 1

I need that.

Speaker 1

No, you have to, you have to do your job flawlessly.

Speaker 1

对。

Yes.

Speaker 1

Sometimes you get gear in the middle of a procedure.

Speaker 1

Like for example, you're scheduled at 8PM or 8AM and a stroke comes out of nowhere.

Speaker 1

All your plan that you just planned for the entire day, you have to scoot to the side because now you have to treat a patient that's having a stroke or a pulmonary embolism or a GI bleed, cold leg.

Speaker 1

Those type of emergency cases trump every single thing that you have planned.

Speaker 1

And then that way you can treat that patient emergently and then you continue on to your regular.

Speaker 1

If you're on call, you do not leave until everything is done.

Speaker 1

So for example, if that case that you just pushed back keeps getting pushed back, the physician has the opportunity to like, all right, we can do this tomorrow or, no, we can't wait.

Speaker 1

This will be done.

Speaker 1

So sometimes it can be a ten hour day.

Speaker 1

Sometimes it could be a sixteen hour day.

Speaker 1

But luckily, it's a whole group, so you're not by yourself.

Speaker 1

And that's one thing that, like, if I was by myself, I would be like, oh, I hate my life.

Speaker 1

But luckily, I have a really good team and we we make it fun.

Speaker 1

We have the really good reps that come in.

Speaker 1

So for example, if we have a very long case and we need very, specific, product, they will come in.

Speaker 1

They'll buy a Starbucks and stuff like that just to keep us going.

Speaker 1

Is life, man.

Speaker 0

Shout out to the team.

Speaker 1

Oh, absolutely.

Speaker 1

是的。

Yeah.

Speaker 1

没错。

Yeah.

Speaker 1

I'm very blessed to have a really good team.

Speaker 1

And that's the thing about IR.

Speaker 1

You get to work with people very closely every single day.

Speaker 1

Some people you're like, man, you know me more.

Speaker 1

You know me better than my husband.

Speaker 1

I'm like, hey, man.

Speaker 1

Just because we work, like, we work so close that we have to trust each other.

Speaker 1

Like, I know for a fact that if I need something, I can trust this person.

Speaker 1

I can trust this person.

Speaker 1

Like, I don't even have to question it.

Speaker 1

I need something.

Speaker 1

They need they do it.

Speaker 1

If the patient's crashing, they know I can I can do my job?

Speaker 1

I can they can do their job.

Speaker 1

It's a level of respect among the entire team that I really, really admire.

Speaker 0

Let's think of your your most interesting case.

Speaker 0

Like, is there a case that maybe stood out to you a little bit more?

Speaker 0

What would be your most interesting case, in interventional radiology?

Speaker 1

So my favorite are, like DVTs and pulmonary embolisms.

Speaker 1

Anything that deal with blood clots because we're able to go in and extract it.

Speaker 1

So this includes inside the legs called DVTs or DVT, thrombosis, pulmonary embolism.

Speaker 1

So we're able to go inside the actual pulmonary arteries and extract all the clots inside of the pulmonary arteries.

Speaker 1

Stroke, we're able to go all the way up.

Speaker 1

We access the femoral artery, go all the way up to the cerebros.

Speaker 1

We're able to suck out the entire clot, reversing the stroke.

Speaker 1

Pretty cool stuff.

Speaker 1

So anything with blood clots and stuff like that is one thing that I have specialized because the tech that taught me, she was very fascinated by it and she taught me everything that she knows.

Speaker 1

So now I have that knowledge and I'm able to teach that for the students that come inside of my hospital.

Speaker 0

Awesome.

Speaker 0

Passing down that knowledge, right?

Speaker 1

没错。

Yes.

Speaker 1

There you go.

Speaker 0

So let's bring it back, right?

Speaker 0

Let's bring it back to, radiology tech school.

Speaker 0

Would you say, or is radiology tech school hard?

Speaker 1

X-ray school, it was in the beginning, it was difficult for me because I worked night shift.

Speaker 1

I was always tired, but my teachers, they they were so motivated.

Speaker 1

They they're very passionate professors.

Speaker 1

They've motivated me to like, hey, you have to learn.

Speaker 1

I don't like, they're like, if you're tired, that's okay.

Speaker 1

Just keep pushing.

Speaker 1

Like they saw the potential in me and I'm very grateful for my professors because at first it was very difficult, but once I saw the passion like that pretty much they hooked me into it.

Speaker 1

And after that, it just became second nature, like the human body, everything, all the positioning, it just came so naturally because of the passion that my professors had.

Speaker 1

So in the beginning it was hard, but after a while, like the physics and stuff like that, I'm a physics background, physics and exposure was like easy for me.

Speaker 1

So even as a student, I understood the material and I was able to teach my fellow classmates like, hey, if the professor wasn't teaching it, where like he wasn't explaining it well, after class I would be like, hey, this is how I do it.

Speaker 1

And that's one thing that in x-ray school, work together and we learn together.

Speaker 1

And now I have lifetime friends for the rest of my life from x-ray school.

Speaker 1

And I really appreciate it.

Speaker 1

It was hard.

Speaker 1

So future students is gonna seem hard in the beginning, but if you put forth the effort, you're definitely gonna pass.

Speaker 1

X-ray school is gonna open so many doors for you.

Speaker 1

Do not give up.

Speaker 1

You don't understand how great of this field how how great this field is.

Speaker 1

Exposure is gonna be a little bit difficult, but don't give up.

Speaker 1

You're gonna have so many opportunities after school.

Speaker 0

And that's facts right there.

Speaker 0

There's a lot of opportunities in radiology, but it does have its challenges, right?

Speaker 0

It does have its challenges.

Speaker 0

So let's talk a little bit about that.

Speaker 0

What would you say are some of the pros and cons of being a radiologic technologist?

Speaker 1

优点?

Pros?

Speaker 1

You will definitely have a better career.

Speaker 1

Healthcare is never going to go away.

Speaker 1

Like all this AI stuff and all that computer, they can't compete against someone.

Speaker 1

When someone's in pain and you have to get an x-ray, x-ray technologist are always gonna be there.

Speaker 1

So this career, you're always gonna have a job anywhere in the world.

Speaker 1

Everybody's gonna break a bone.

Speaker 1

This this field is always gonna be in demand.

Speaker 1

Some of the cons, sometimes it's just like long hours.

Speaker 1

It depends on where you work.

Speaker 1

Like some facilities only do certain amount of cases.

Speaker 1

Some facilities only do limited studies and stuff like that.

Speaker 1

So like trauma one hospitals, you're going to be really, really hectic, work hours.

Speaker 1

So you just got to look at what you want to do.

Speaker 1

If you have a family clinic life is more better because you have weekends off, holidays off, stuff like that.

Speaker 1

That's the only thing that I see as a con.

Speaker 1

Like you have to be flexible with your time.

Speaker 0

So in your opinion, would you recommend radiology or in your opinion, do you feel radiology is worth it?

Speaker 1

I 100% think, radiology is worth it.

Speaker 1

Before this, I was in restaurants and construction and I hated myself every single day.

Speaker 1

Like I used to work in Walmart, like while I was in X-ray school, like overnight sometimes.

Speaker 1

Like, oh my God.

Speaker 1

It's just, once I passed my X-ray boards, it opened so many doors.

Speaker 1

Now I'm able to learn, be with people that I love.

Speaker 1

X-ray school really opened the doors.

Speaker 1

Radiology is one of the best fields.

Speaker 1

It's a field that's growing a lot more.

Speaker 1

Now for example, your podcast, like my YouTube channel, your YouTube channel, like more people are seeing what radiology is because a lot of people are like, oh, they're just button pushers.

Speaker 1

No, no, no, no.

Speaker 1

The field of radiology is so is growing every single day.

Speaker 1

Technology is getting better, like MRIs, ultrasound, Nukemed, all that falls under radiology.

Speaker 1

You have so many different doors that open once you get your x-ray license.

Speaker 1

Honestly, it's going to be life changing.

Speaker 0

That facts right there.

Speaker 0

And let me ask you this, right?

Speaker 0

Because CT, MRI are very popular, right?

Speaker 0

X-ray is very popular.

Speaker 0

I think interventional radiology goes under the radar a little bit.

Speaker 0

So to get into interventional radiology, can you still do that under your associates or do you need a bachelor's in order to get into interventional radiology?

Speaker 0

We'll be back after a quick break.

Speaker 0

Some of the coolest radiology shirt, under scrubs, notebooks, and stickers are available at my website, yourxraytech.com.

Speaker 0

Check it out.

Speaker 1

All you need is your associate, x-ray associate.

Speaker 1

No bachelor's required.

Speaker 1

Some, some hospitals might require it, but honestly it's just there's no formal education for interventional radiology because like you said, it's very not well known.

Speaker 1

Even when I was a student, I didn't even know about it.

Speaker 1

It was funny how I got introduced to interventional.

Speaker 1

Like I was a student, I was just chilling in the break room and one of the techs, he's now my manager, he's like, hey, I need someone to run a C arm.

Speaker 1

And none of the techs were like, nah, I'm I'm too busy.

Speaker 1

And I was a I was just a student.

Speaker 1

I was like, I'll go.

Speaker 1

And then that's how I was introduced to Interventional Radiology.

Speaker 1

I didn't even know about it.

Speaker 1

And the first thing I did was, two C arms at the same time doing biplane, kyphoplasty, which is, sticking, rods inside someone's vertebral bodies to put cement in order to reverse compression fractures.

Speaker 1

That's what a kyphoplasty is.

Speaker 1

And I was like, wow, this is what you guys do?

Speaker 1

He's like, you got you don't even know that this is the tip of the iceberg.

Speaker 1

So then I started to keep I started going and going.

Speaker 1

And that's how I got introduced to interventional.

Speaker 1

That's one thing that I want to do, like more awareness of interventional radiology, because a lot of people don't know what we do.

Speaker 1

Even in my hospital where I work at, the CEOs or top management stuff like that, they don't even know what we do.

Speaker 1

They're just like, Oh, they just send them to IR and they come back fixed.

Speaker 1

They don't know what we do, but with your help, we can spread awareness of interventional radiology because this is a field that's very interesting and a lot of people should know about more.

Speaker 0

So for some people, this is the first time hearing about interventional radiology right here on yourxraytech podcast.

Speaker 0

And the information's coming from Lazy Bones Radiology himself, right?

Speaker 0

So this is straight from the source right here.

Speaker 0

I mean, he just came from a shift right before the podcast.

Speaker 1

Yeah, I love this podcast.

Speaker 0

没错。

There you go.

Speaker 0

So it's as real as it gets, right?

Speaker 0

So you talked about how you got into interventional radiology.

Speaker 0

How did you get into radiology in general?

Speaker 0

Like, what made you decide I'm going to take this path?

Speaker 1

Funny story.

Speaker 1

I broke my arm at work.

Speaker 1

And so I had to get multiple x rays, MRIs, CTs of my arm because I broke it right at the greater tuberosity.

Speaker 1

So I needed a lot of imaging and stuff like that.

Speaker 1

Every time I had to get it like an image, was like, what is that?

Speaker 1

Is that broken?

Speaker 1

And like the techs would be like, sir, we can't tell you.

Speaker 1

We're not the physician.

Speaker 1

I was just like, come on, you can tell that.

Speaker 1

Like how everybody does it.

Speaker 1

But that's what interests me into radiology.

Speaker 1

For two years, I couldn't move my arm.

Speaker 1

So I had to get multiple imaging and stuff like that.

Speaker 1

That's what promoted me into the field of radiology and where the college that I was at, they had the x-ray program.

Speaker 1

So I was like, I'm interested in the medical field.

Speaker 1

I want to be a doctor.

Speaker 1

Let's just get, let's see what this x-ray thing is about.

Speaker 1

That way I can get my foot in the door.

Speaker 1

That was one of the best decisions I've ever made because now I work with some of the top tier physicians in the world.

Speaker 1

They do live cases that are presented in Dubai, some in Brazil, Portugal, New York, all throughout The United States.

Speaker 1

Here in Florida, in the interventional radiology world, we're one of the biggest in the world.

Speaker 1

Like they go into conferences worldwide.

Speaker 1

I'm in the hospital that some of the best physicians and I have the opportunity to learn.

Speaker 1

And I have the opportunity to have mentors like them just because of one broken arm.

Speaker 1

It's funny every time I say it, I'm like, man, if I would have not broken my arm, I would not be in this profession right now.

Speaker 0

That's interesting.

Speaker 0

Something came out of an unfortunate event.

Speaker 1

Exactly, man.

Speaker 1

Made some silver lining.

Speaker 0

Silver lining.

Speaker 0

And the arm's good now, right?

Speaker 0

You're good arms.

Speaker 0

Oh, you.

Speaker 1

I'll keep my arms perfect.

Speaker 0

There you go.

Speaker 0

And I'm sure the physicians are glad that you're on board too, and they get to do their part, because you're providing the images and everything else that comes with it.

Speaker 0

So, again, that's why I like to highlight our field because I don't think like a lot of the heroes go under the radar, right?

Speaker 0

And most are okay with that.

Speaker 0

They go to bed okay with, I did all this and nobody really knows, but it's okay.

Speaker 0

So we're just shining a little light on the everyday things, right?

Speaker 1

All the techs of every single field, you guys should be proud.

Speaker 1

Radiology, we are the eyes of the medical field.

Speaker 1

Without us, they can't really see.

Speaker 1

Like no matter what physician there is, without imaging, they will not be able to do their job.

Speaker 0

Interventional Radiology is one of the paths, right?

Speaker 0

So you went from x-ray to interventional radiology, and you're credentialed under interventional right?

Speaker 0

You have that in your name.

Speaker 0

So can you sum up or give us a summary version of how you go from an x-ray tech to an interventional, radiology tech?

Speaker 1

Once you pass your x-ray school, you take your registry.

Speaker 1

For IR, you have to do a total of almost 200 cases.

Speaker 1

You have to document.

Speaker 1

You can do categories.

Speaker 1

It just depends on what, what you, what cases you have available, but you have to document 200 cases.

Speaker 1

I went through ASRT.

Speaker 1

They have a online 12 course, like 12 program course that you have to pass in order to have your formal education.

Speaker 1

And then after that, once you complete all that, you take your board.

Speaker 1

Like there's no schooling for it, but it's on the job training.

Speaker 1

So you have to take a call.

Speaker 1

You have to do all the things that you have to do.

Speaker 1

You have to learn fast.

Speaker 1

One thing about Interventional Radiology, it's not easy.

Speaker 1

It's like jumping into the deep end.

Speaker 1

There's no little, oh, let me dip my toe in here.

Speaker 1

No, they throw you in there.

Speaker 1

You have to survive.

Speaker 1

You have to know what you're doing.

Speaker 1

You have to adjust.

Speaker 1

Okay, this is what you gotta do.

Speaker 1

Take lots of notes, learn from the nurses, learn from the physician.

Speaker 1

That's one thing you have to do.

Speaker 1

You have to be able to work as a team because if you work by yourself and try to study, it's just so much knowledge is is is pretty much gonna be you're you're destined to fail if you don't ask for help.

Speaker 1

So I like, when I entered, they'll you have to know pharmacology.

Speaker 1

So I asked all the nurses, hey.

Speaker 1

How did you remember this medication?

Speaker 1

And they will teach me their little their tricks and their ways to doing it.

Speaker 1

The physician, why would he use this catheter?

Speaker 1

And he is like, oh, we need to use this because of this.

Speaker 1

Like, the more you build, the better it is, especially for this field, interventional, because there's so much information you have to know.

Speaker 1

The more you're able to like, for example, in a case, we can use 50 different, products.

Speaker 1

If you don't know what different, products you have, you're gonna be lost.

Speaker 1

But if you associate it, okay, this anatomy goes with this.

Speaker 1

Oh, I had to use this for that.

Speaker 1

Like you're, you're connecting all these little details into one big picture.

Speaker 1

And now all that information is easier to understand.

Speaker 0

Great answer.

Speaker 0

对。

Yeah.

Speaker 0

So, so, but very much worth it, right?

Speaker 0

Like all that.

Speaker 0

Very much worth it and very possible too.

Speaker 0

Right?

Speaker 0

The common question I get is pay, right?

Speaker 0

About pay.

Speaker 0

And I always say, you know, you should choose these fields because you want to help people and for all the right reasons.

Speaker 0

Yes, pay is important, but it shouldn't be the main thing.

Speaker 0

Is there a difference in pay from doing x-ray and then moving on to interventional radiology?

Speaker 1

Yes, there was a significant, pay increase.

Speaker 1

We get more per call.

Speaker 1

Hourly, my rate went up really good.

Speaker 1

And also because of the call, you also get a lot of bonuses on that.

Speaker 1

Like for example, like if I'm on call right now, you get a certain amount of pay just for being on call.

Speaker 1

And then once you get called in, you get either time and a half or double time in some in some facilities.

Speaker 1

Yes, you get a lot of pay, but you have to work for it.

Speaker 1

Because one thing about IR, like I said, is a group.

Speaker 1

And if the group does not accept you, like if you're there just for the money, they'll they'll they'll fish you out and they'll kick you out because we're here to help.

Speaker 1

We're here to be for our patients.

Speaker 1

So yes, the money's there, but you have to be willing to work and you have to be there for the patient because if you're there for the money, yeah, you're going to be there collecting the check, but you're not going to make a difference.

Speaker 1

And if the physician sees that, some physicians, they'll I don't want to work with a tech.

Speaker 1

If they see that you have an agenda, they'll kick you out because we're here to help patients.

Speaker 1

But yes, we do get paid good.

Speaker 0

不错。

Nice.

Speaker 0

Great answer.

Speaker 0

Great answer.

Speaker 0

Speaker 1

If I had to do it again, I'll do it again.

Speaker 1

And Interventional Radiology is one of the best.

Speaker 1

The SCT, MRI, everything else is also good.

Speaker 1

But IR is my heart.

Speaker 1

It's what I do every day.

Speaker 1

But definitely, if you guys are interested, Interventional Radiology is one of the best fields.

Speaker 1

You're gonna see a lot of crazy stuff.

Speaker 1

And if you are able to shadow, ask your clinical instructors if they can shadow an interventional radiology and see if you like it.

Speaker 1

Good luck on everything.

Speaker 1

Don't forget to study.

Speaker 1

Don't forget to do your homework and know all your positionings because that's definitely a big part of every single field.

Speaker 1

The positioning, you have to know your anatomy, all your physics and stuff like that.

Speaker 1

Not so much, but it's very important.

Speaker 1

And that's it.

Speaker 0

Awesome.
